چکیده
Dear Editor Study done by Bista et al in their article entitled “Tonsillar microbial flora: A comparison of infected and non-infected tonsils” published in Kathmandu University Medical Journal 2006, volume 4, number 1, issue 13 is outstanding. However we want to add certain points which need to consider in establishing causative organism and treating patients with tonsillitis with some healthy criticism. In the study, authors have taken 32 cases and 18 controls and are not clear whether the age matching was done in case and control groups. In a study done by Brook et al in 1995 comparing microbial flora of adults and children have findings suggesting that the etiology of recurrent tonsillitis in adults may differ from that in children. The differences in the tonsillar flora may be due to the effect of many more courses of antimicrobials given over the years to adults and the changes in tonsillar tissue that occur in this age group. Similarly, it is important to note that the causative organism may differ in acute and recurrent tonsillitis and also in otherwise healthy children as compared to always ailing children ( Marushko et al 1999).Their studies have shown development of dysbacteriosis, decrement of tonsil colonization resistance in always ailing children. It is Staphylococcus aureus which is more commonly isolated in these children as compared to the normal. Furthermore, several studies have shown that there is strong association between H. influenzae and recurrent acute tonsillitis and tonsillar hypertrophy. This is a fact which needs to be considered in designing and implementing therapeutic measures.

OBJECTIVE The main objective of this study was to find out the most common organism affecting the tonsils in recurrent tonsillitis. METHODOLOGY This was a prospective study consisting of total number of 50 patients, conducted in the ENT department of Dhulikhel Hospital, KUTH, Dhulikhel, Nepal. The study period was one year and three months (March 2004 to April 2005). RESULTS Total numbers o...
متن کاملDetection of lymphocytes productively infected with Epstein-Barr virus in non-neoplastic tonsils.
Epstein-Barr virus (EBV) persists for life in the infected host. Little is known about EBV reactivation and regulation of virus persistence in healthy individuals. We examined tonsils of chronic tonsillitis patients to detect EBV transcripts, EBV genomes and lytic proteins. LMP1 transcripts were observed in 11 of 15 specimens and BZLF1 transcripts were detected in six. Multiple copies of EBV ge...
متن کاملEvidence of Bacterial Biofilms among Infected and Hypertrophied Tonsils in Correlation with the Microbiology, Histopathology, and Clinical Symptoms of Tonsillar Diseases
Diseases of the tonsils are becoming more resistant to antibiotics due to the persistence of bacteria through the formation of biofilms. Therefore, understanding the microbiology and pathophysiology of such diseases represent an important step in the management of biofilm-related infections. We have isolated the microorganisms, evaluated their antimicrobial susceptibility, and detected the pres...
متن کاملComparative investigation of the culture of palatine tonsils by superficial and deep methods and investigation of microbial resistance in frequent tonsillitis patients referred for tonsillectomy
Introduction: The tonsils are part of the immune system and tonsillar infection is one of the most common causes of diseases, especially in children. Tonsillitis sometimes does not respond to empirical treatment and the presentation of recurrent tonsillitis eventually leads to tonsillectomy. The aim of this study was to determine and compare palatal tonsil culture by superficial and deep method...
